***UPDATE: Location has been determined.  The event will be at the UCSD Thornton campus, in the East Campus office building, Room 1-001 (just inside the door).  I have attached a map and an image of the building for your reference.  You will need to pay for parking - the ER (Campus Point) parking garage is closest; it's an easy walk from there.  Parking fees will be estimated around $4.00 to $6.00.***

It's been a while since HPNA had a presentation on Advance Care Planning DIscussions.  Join us on July 11 for an Advance Care Planning Workshop with John Tastad, M.A., a locally recognized expert in advance care planning discussions, goals of care conversations, and POLST.  John provides consultations for Sharp's most complictated cases and sits on committees for bioethics and for EOLOA implementation at Sharp Healthcare.

This workshop will help you practice skills of goal setting with your patients.  This forum is an ideal setting to brush up on your skills or for employees who are new to hospice and palliative care.

Learning goals:

  • Articulate the importance and key elements of advance care planning conversations
  • Verbalize the primary purpose and use of the POLST form and how to skillfully engage in POLST conversations
  • Verbalize phrases that are helpful in Goals of Care conversations
  • Articulate the components of California’s End of Life Option Act

We believe everyone will find great value in this workshop! All nurses who work in hospice and palliative care or advance care planning are welcome to attend.  Nonmembers are requested to donate $10 for the cost of dinner and CE credit.
